2. All fields marked * are required.
3. You will be contacted by the Columbus Film Office regarding your application. From there, you will be directed to the appropriate municipality permitting office(s).
4. After submitting this application, specific location permits may be requested via the corresponding municipalities’ permit applications. If approved, an agreement will be finalized between a Film Production Coordinator and a Permitting Office Liaison.
5. This application is for Professional Productions only. Students do not need to register with the Columbus Film Office’s Project Registration Form.
